Please Note: Patient I.D./Your PO Number will be used to identify your items to be fabricated. After completing your order, attach Patient I.D./Your PO Number to your digital scan or cast.

Digital scans can be emailed to info@fabcoprostheticdesigns.com and if a cast or negative model is being shipped to FabCo, please attach your Patient I.D./Your PO Number to the cast or negative model so we can match against PO from SPS.

Omega Software Users:

Select "Carving Only" in Fab stack and select FabCo Prosthetics Designs. Make sure to add Patient I.D./Your PO Number as your PO in Omega so we can match against PO from SPS.

What will I be receiving?

For 3-Prong Adapters and 4 Hole Laminating Adapters, you will receive the fabricated check socket, the adapter, and a 2 inch roll of Techform casting tape.

For Bulldog Locks, you will receive the fabricated check socket, a lock, and (2) 1.25 inch plunger pins.

For Bulldog Lanyards, you will receive the fabricated check socket, the lanyard puck, and a 1 inch chafe attached anteriorly, unless otherwise specified in notes.

For Valves, they will be placed distally, at anterior/medial corner, unless otherwise specified in notes.

Warranty Info:

Warranty: This item is warranted for 15 days from the invoice date.

This item is fabricated with information provided to FabCo by the Prosthetist, complete and accurate information is necessary to provide the patient with a quality socket. FabCo offers a 15 day warranty on custom fabricated devices for poor craftsmanship.
